Formbricks on your own server, in one click.
Run surveys and forms with Formbricks on a server you own, and share them as links or show them on your website or in your app. ServerAvatar runs it in Docker with its own PostgreSQL and Valkey and sets up the secrets, storage and HTTPS for you.
- Formbricks + PostgreSQL
- HTTPS on its own
- Your server, your responses
Section 01What is Formbricks
What is Formbricks?
Formbricks is an open-source platform for surveys and forms. You build a survey in its editor, share it as a link or show it on your website or in your app, and read the answers in a summary for each question.
Running it on your own server keeps every response in your own database. The free Community Edition has unlimited surveys, responses and users in one workspace; a paid Enterprise licence adds features such as teams and access roles, SSO and contact segments.
- Free and open source (AGPLv3)
- Link, website and in-app surveys
- Unlimited surveys, responses and users
Section 02Features
What you can do with Formbricks
Link surveys
Share a survey as a link or a QR code, embed it in a web page or an email, and pass details such as a user ID in hidden fields.
Website and app surveys
Add the Formbricks SDK to your website or app and show a survey on a page view, a click, exit intent or a 50% scroll.
Every common question type
Free text, single and multiple choice, picture choice, rating, NPS, ranking, matrix, date, address, contact info, consent and more.
Logic and recall
Show or skip questions with conditions, actions and variables, and repeat an earlier answer in a later question.
Multi-language surveys
Translate one survey into several languages, so each respondent answers in their own.
Summaries and exports
See a summary for each question, filter responses, download them as CSV or Excel, and send them on with webhooks, Zapier, Make or n8n.
Section 03Requirements
What you need
A fresh Ubuntu server you can log in to as root. The installer checks all of this before it changes anything.
| Requirement | Formbricks on Docker |
|---|---|
| Operating system | Ubuntu 24.04 or 26.04 LTS (x86-64) |
| CPU | 1 core or more |
| RAM | 2 GB or more |
| Disk | 15 GB or more (25 GB recommended) |
| Containers | Formbricks 5.4 + PostgreSQL 18 (pgvector) + Valkey 8 |
| SMTP, for invitations, password resets and follow-up emails | |
| Ports | 80 and 443 open |
CPU and RAM are the minimum in Formbricks' docs for version 5.4, which the recipe runs; the docs for version 6 ask for 2 vCPU and 4 GB for its larger eight-service stack. Disk: about 9.5 GB is in use once the panel and Formbricks are installed on our test server; 25 GB is Ubuntu's recommendation.
Section 04Install
How to install Formbricks
Every step happens in the ServerAvatar panel. No terminal needed.
Pick Formbricks in 1-Click Recipes
On your server, open Applications, click Create application and search for Formbricks under 1-Click Recipes.
Name it and choose a domain
Give the app a name and use your own domain, or a temporary one that works right away. The panel creates a system user for it.
Create it
Click Create application. The panel starts Formbricks, PostgreSQL and Valkey, turns HTTPS on and shows the secrets it generated. Copy them somewhere safe. The first start takes about half a minute while Formbricks sets up its database.
Create the administrator and your organization
Open the app's domain straight away, because the setup is open to the first visitor. Click Get started, fill in Create Administrator with your name, email and a password, then name your organization. That account becomes the owner. Public sign-up stays closed from then on: add people under Organization settings > Teams.
